The short version

This website has no advertising trackers, no analytics cookies, no session recording, and no third-party embeds. Nothing you do here is shared with advertising platforms. The only information we receive is what you deliberately type into the consultation request form.

What the request form collects

When you submit a consultation request, we receive only these fields: your name, your phone number, your email address if you choose to give one, who the visit is for, which treatment you are interested in, the best time to call, how you heard about us, and whether you have dental insurance (yes or no only — we never ask for your insurer or member number). We also record which campaign link brought you here, if you arrived from one.

The form deliberately has no message box and asks no medical questions. Please do not include health details in it.

What we do with it

We use it to call you back about scheduling a consultation, and for nothing else. We do not sell it, rent it, or share it with anyone outside the practice. We do not use it for advertising.

Requests are delivered to a private spreadsheet insideBeaver Lake Orthodontics’s Google Workspace account, which is covered by a HIPAA business associate agreement, and are visible only to named staff.

What this site does not do

• No advertising or social media pixels, including the Meta Pixel.
• No Google Analytics, tag manager, or conversion tracking.
• No session replay, heatmaps, or chat widgets.
• No cookies used to identify or follow you.
• No selling or sharing of any information, ever.

We do run paid advertisements on Instagram and Facebook. Those ads link here with a plain campaign tag in the address. No tracking code from those platforms runs on this website, so they are not told what you do once you arrive.

Email

Email is not a secure way to send health information. Please do not email medical details. If you need to discuss something clinical, call us at 479-633-7993.

Children

This site is written for parents and adults. If you are under 18, please ask a parent or guardian to submit the form for you.

Your choices

You can ask us what request information we hold about you, ask us to correct it, or ask us to delete it. Call479-633-7993 or emailinfo@beaverlakeortho.com.
